Health Passport joins GTC
In order to expand their reach into the global market place, Health Passport has recently joined the Global Team Care network. Health Passport provides a series of treatment programs for the prevention and management of diabetes and related chronic diseases. The Health Passport series is focused on early detection of diseases and health risks and the use of treatment programs and applied patient education to involve patients proactively in their pursuit of improved and sustained health.
The programs in the Health Passport (HP) series are interrelated, yet are also effective as individual solutions. Two of the programs that Global Team Care is particularly excited about are Predictive Assessment and Preventive Response. These two programs compliment each other and add value to both sides of the GTC referral management platform.
Predictive Assessment provides and organized methodology for screening patients for chronic diseases such as diabetes and heart disease. If the patient is diagnosed as having or being at risk for a disease, the Predictive Assessment providers can refer them for treatment through HP and GTC as well. Preventive Response offers specialized and customized programs to reduce the risks or symptoms of these diseases and patient education during the program for a healthy transformation of lifestyle choices.
